SPECIAL INSTRUCTIONS FOR ORDERING DOG TAGS ------ PLEASE READ

 

We tried not to make this dang form too complicated, but stuff happens!  This order button will allow you to order either style of dog tags, pre-1964 and post-1964.  The form also will allow you to put your last name and first name with MI.  Hopefully, these buttons will work fine and there will be no problems! 

 

Now here's the complicated part ------------ when you hit check out, there is a section for 'Special Instructions for the Seller' (us).  In this section you need to enter your SSAN (or old service number), blood type and religious preference.  Please abbreviate as much as possible -- if anything is missing or incomplete, we'll contact you before the dog tags are made.

 

Official dog tag ID set includes two rolled edge stainless steel embossed plates with a short and long stainless steel beaded chain.

Below, enter your last name and first name, MI.  When you click 'Add to Cart' you will be prompted in a 'Special Instructions to Seller' block to add your blood type, SSAN (or old serial number), and religious preference.  These three items are optional on your part.  Please let us know if you want anything left out.  If, by chance, information arrives incomplete, we will contact you by e-mail to fill in the blanks.  Your privacy is assured - we keep no record of this information!
